Acne Vulgaris Consists Of Lesions

Once a boil becomes soft or “forms a head” (that is, a small pustule is noted in the boil), it can be ready to drain. Once drained, pain relief can be dramatic. It is larger than a pustule, may be severely inflamed, extends into deeper layers of the skin, may be very painful, and can result in scarring. Cysts and nodules often occur together in a severe form of acne called nodulocystic. The physician needs to determine if the condition is non-inflammatory (open and closed comedones), inflammatory (papules or pustules) or a mixture of both (the most common situation). Topical treatment is sufficient in most patients with acne, but systemic therapy is required in patients who have deep acne with nodules and cysts.

Most people when they think about acne, they typically just think about pimples, whiteheads and blackheads, and/ or minor blemishes. The fact of the matter though is, acne can actually progress beyond just those symptoms. There are going to be non-inflamed blackheads and whiteheads but surface of the skin is getting slightly inflamed. The frequency of pimples appearance and their breakout activity will be more obvious. Acne “cysts” do not have an epithelial lining; they are composed of poorly organized, variously shaped and blackhead or whitehead. Alternatively, the microcomedo may become an inflammatory lesion, such as a papule or pustule.

Blackheads, whiteheads, and inflamed pimples are surface blemishes. As such, they are relatively simple to control. Cystic acne is more serious than your common pimples, whiteheads or blackheads. This form of acne is rooted down to the deep layers of the skin. The doctor will go in and literally remove cysts, blackheads and whiteheads. There is a risk of infection during this surgery and it should only be considered in severe cases.

Yet another method used to treat cystic acne involves surgery to remove visible blackheads and whiteheads, if present. A pointed instrument is inserted in each legion to loosen it against its own resistance and expose its contents, which are removed. Acne is moderate when pimples (blackheads, whitehead, and comedones) cover approximately half of the face. If you have moderate acne, consult a dermatologist in order to quickly get a handle on your situation. These are bigger than the small whiteheads or pimples of other varieties of acne vulgaris (common acne) although like those pimples, they usually contain white pus.

The interlesional corticosteroid injection not only helps in reducing the inflammation, but promotes the healing process as well. The injection works by melting the cyst over a period of time, until it has dried out. This seems to be the only way the lesions ever heal – if I do not pop them they stay under the skin as a lump for months. Deeper breaks in the follicle wall cause more serious lesions. Nodules develop when the contaminated material infects adjoining follicles.

I even use it AM and PM on the areas where I have blackheads and I am amazed at how they are disappearing. I use this along with the Drying Lotion and my skin has never looked this good. My favorite prescription topical that shows consistent resolutions of blackheads continues to be Retin A .025% cream. Frequently, patients will tell me they have used Retin A in the past and have experienced quite a bit of irritation.
